What P0491 usually means
P0491 is a mainstream emissions code because secondary air systems fail in recognizable ways on BMW, some GM products, and other OBD-II vehicles that use cold-start air injection. The code means the controller expected extra oxygen to reach bank 1 during the air-injection test but the exhaust response was weaker than expected. That does not always mean the air pump itself is dead. Combination valves, clogged passages, vacuum control issues, moisture-damaged pumps, and cracked hoses all regularly cause the same insufficient-flow result. Cold-start timing matters here because a car can run perfectly warm and still fail the monitor every morning.
